Who receives free NHS maternity care in England and when can charges apply?

Continue care during status checks
Contact maternity services as soon as pregnancy is established. All antenatal, intrapartum and postnatal maternity services are immediately necessary and must not be delayed or withheld because of charging status or lack of money. This does not make otherwise chargeable treatment free. GOV.UK: maternity services and charges ↗.
If booking is linked to advance payment, ask the maternity manager and overseas visitors manager to review the arrangements. Seek immediate help for urgent symptoms.
Request an individual assessment
Ask the hospital's overseas visitors team which evidence is needed. This may involve lawful settled residence, a valid visa and IHS entitlement, protection status or another exemption. Particular Ukrainian and other migration circumstances have specific provisions, assessed on the facts.
Paying IHS, or a relevant exemption under an appropriate visa, can provide access on a similar basis to ordinary residents. A visitor visa does not itself create that entitlement. Use current charging guidance ↗, rather than another family's experience.
Obtain the decision and calculation
If assessed as chargeable, request the legal basis, services covered and written calculation. Chargeable overseas visitor care generally uses 150% of the NHS tariff, which differs from a private maternity package. Actual care and complications affect cost.
Current guidance states that maternity charges and debt collection should not take place before delivery, with a separate provision for leaving the UK before delivery. Care must not be delayed by a dispute. GOV.UK: maternity charging ↗.
Address errors or payment difficulties
Supply missing documents and request reassessment if the decision appears wrong. PALS can assist communication. Tell the trust if payment is difficult and discuss arrangements; lack of money is not a reason to miss care.
Check the baby's entitlement and postnatal arrangements separately: an NHS number does not replace assessment. Seek qualified individual advice about immigration implications of debt. Scotland, Wales and Northern Ireland have different rules from England's described here.
